Job Summary
The Registered Nurse (Weekend On-Call) provides hospice nursing care as a case manager and directs clinical care within patients' homes and communities. Builds from community resources to plan and deliver services to patients and families. Responsibilities include case managing the interdisciplinary team, implementing the patient plan of care, ensuring infection control and documentation standards, monitoring cases for payer compliance, and supervising hospice aides and LPNs. On-call duties are scheduled Friday 4:30pm through Monday 8:00am. Requires RN licensure, CPR, and a valid driver’s license with insurance. Strong communication, observation, problem-solving, and nursing skills are essential; ability to lift and transfer patients and equipment is required.
Required Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State license as a Registered Nurse
- Current CPR certification
- Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements
- Excellent observation, verbal and written communication skills
- Problem solving skills, basic math skills; nursing skills per competency checklist
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Case manages and provides clinical direction to the interdisciplinary team, physician, and family
- Provide care utilizing infection control measures that protect both staff and patient (OSHA)
- Monitor assigned cases to ensure compliance with third-party payer requirements
- Demonstrate commitment, professional growth, and competency
- Promote agency philosophy and administrative policies
- Perform on-call responsibilities and provide on-call service to patients/families as assigned
- Responsible for supervising hospice aides and licensed practical nurses
