Registered Nurse - Weekend

As an RN in home health, provide skilled nursing care in a patient’s home under the direction of an attending physician. You will work autonomously to manage time and duties within the scope of practice, perform OASIS assessments to develop and modify individualized care plans, administer medications, and minimize patient risk. You’ll collaborate with an interdisciplinary team, educate patients and families using teach-back methods, and support transitions of care including admissions, transfers, and discharges. The role includes scheduling responsibilities with a weekend rotation (2 weekdays and every other weekend, 8:15a-5p, with on-call). You’ll be part of a team that emphasizes compassionate care, high-quality outcomes, and patient-centered care in the home setting, with opportunities to engage in documentation and compliance with Medicare/Federal guidelines.
