Job Summary
Conduct home visits to assess patient needs and provide necessary medical care in Worcester, Shrewsbury, Boylston, and West Boylston. Utilize Case Management, Oasis E documentation, and EMR systems to ensure accurate tracking of patient progress and treatment plans. Collaborate with patients, families, and interdisciplinary team members to develop and implement individualized care plans. Requires a valid MA RN license, minimum one year of med/surg experience, and willingness to travel within the designated territory. Full-time schedule includes three to five days, 8a-2p or 8a-4:30p.
Required Qualifications
