Job Summary
Conduct home health visits to administer IVs, change dressings, clean wounds, and perform comprehensive patient assessments. Document all findings, update physician orders, and upload records to the Synergy Software portal. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise patient care plans while coordinating with physicians and other team members regarding significant condition changes. Perform re-certifications, transfers, and discharges as requested, and supervise Home Health Aides per regulatory requirements. Requires current CA RN license, BLS certification, IV/Phlebotomy skills, and ability to work two-three days weekly with weekend/holiday coverage.
Required Qualifications
- 1 year RN experience
- 1 year nursing experience
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
- Ability to work at least two-three days each week
- Ability to work one weekend a month
- Ability to work two holidays per year (one winter and one summer)
- Knowledge of regulatory requirements
