Job Summary
Complete initial and ongoing patient assessments, develop comprehensive care plans with family involvement, and administer prescribed medications and treatments. Coordinate discharge planning, counsel patients on health instructions, and communicate regularly with physicians regarding condition changes. Manage assigned caseloads as a Case Manager when directed, supervise home health aides, and ensure necessary equipment and services are available. Participate in on-call duties as defined by policy. Requires current RN licensure, CPR certification, and at least one year of home health experience. Visit rates range from $21.00 to $140.00.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state
- Maintains a current CPR certification
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Minimum of one year visiting home health care experience
- Self directed and able to work with minimal supervision
- Demonstrates excellent observation, problem solving, verbal and written communication skills

- Prolonged or considerable walking or standing
- Able to lift, position, or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling, or crouching
