Registered Nurse-VHS-Hospice-FT (Bergen, Passaic & Hudson Counties)

Provide professional nursing care to patients in their homes through assessment, development, implementation, and evaluation of individual care needs. Assure compliance with physician orders, complete medication reconciliation, and update care plans within two weeks of status changes. Act as clinical supervisor of home health aide staff, conduct supervisory visits, and collaborate with the interdisciplinary team to coordinate services and referrals. Identify appropriate payors, maintain visit frequency with authorizations, and report patient care problems to management with proposed solutions. Communicate pertinent patient information during care load transfers and complete routine assignments independently to meet productivity requirements.
