Job Summary
Registered Nurse (RN) triage and care coordination in an oncology clinic. Responsibilities include remote nursing assessments, care planning, patient education, and coordination with multidisciplinary teams to support transitions of care. Requires current RN license and a BSN with obligation to obtain BSN within four years of hire. Skills include critical thinking, motivational interviewing, effective communication, time management, patient education, and proficiency with Microsoft Office. Location: Intermountain Health Lutheran Hospital, Wheat Ridge, CO. Schedule: 40 hours/week, on-site.
Required Qualifications
- Current Registered Nurse (RN) license in state of practice
- Bachelor of Science in Nursing (BSN) from an accredited institution
- RNs hired or promoted into this role must obtain their BSN within four (4) years of hire or promotion date
- Demonstrated clinical nursing experience in chronic disease management, and familiarity with chronic disease terminology and processes
- Basic computer skills and knowledge of Microsoft Office software
