Job Summary
Assess patient physical and emotional status, medical history, and home environment to develop comprehensive nursing care plans. Serve as patient advocate by explaining rights, grievance procedures, and family responsibilities while involving families in care planning. Participate in updating interdisciplinary care plans biweekly and supervise Hospice Aide/CNA care per guidelines. Monitor treatment reactions and immediately inform the physician or Patient Care Coordinator of any changes. Requires RN licensure with hospice or home health experience preferred.
Required Qualifications
- Registered Nurse licensed in the state of operation
- Nursing experience
- Sound knowledge of nursing principles, procedures and elements of patient family teaching
- Must be able to obtain a medical history and assess the patient's physical and emotional status, level of competency, home environment, safety factors, family or household member's ability to assist with care and the needs of the patient and what teaching will be required
- Must be able to serve as the patient's advocate by providing information on patient rights, grievance procedures, and reimbursement procedures
- Must be able to participate in updating the interdisciplinary care plan every two weeks
- Must be able to supervise and direct the care given by the Hospice Aide/CNA
- Must be able to observe signs, symptoms and reactions to treatments and inform the attending physician and/or Patient Care Coordinator immediately of any changes
