Registered Nurse - Transplant Care Coordinator - RN: Post-Heart Peds/Adult

Coordinate all aspects of care for the transplant patient population by assessing patient status, interpreting age-specific data, and collaborating with the multidisciplinary healthcare team to plan care across the continuum. Provide consistent patient and family education while leading community involvement, committee work, and participation in quality improvement and research. Adhere to ANA Scope and Standards, Code of Ethics, UNOS, and FACT guidelines. Requires state licensure and VisaScreen certification; eligible for TN sponsorship.
