Job Summary
Deliver comprehensive patient-centered care addressing physical, emotional, and social needs for homeless and at-risk individuals. Conduct rapid triage assessments and educate patients on managing chronic and acute conditions such as diabetes, hypertension, and wound care. Administer medications safely, facilitate access to housing support services, and oversee care for patients with transitional and complex health conditions. Provide case management, crisis services, and medication-assisted treatment counseling as part of a Substance Use Disorder team. Mentor new nurses and nursing students to enhance their clinical skills. Work day, evening, or weekend clinics as scheduled.
Required Qualifications
- AD, BSN, or MSN in nursing from an accredited college or university
- Current licensure from the State of Colorado to practice as a Registered Nurse
- Certification in Basic Life Support
- Successful completion of privileging and credentialing process for RNs at the Coalition
- Valid driver's license
Desired Qualifications
- Preferred experience in ambulatory, public, or community nursing
- Preferred experience in mental health, emergency room, urgent care, palliative, care management, or critical care
