Registered Nurse - Transitional Care

Job Summary

Coordinate health care transitions for patients by collaborating with partner hospitals to improve discharge outcomes and follow-up access. Conduct rapid triage assessments, administer medications safely, and educate patients on managing chronic and acute conditions. Provide skilled nursing care, case management, and crisis services within an integrated, trauma-informed model. Mentor new nurses, document all interactions in the EHR, and participate in quality improvement initiatives. Work 50% clinic and 50% field-based, including home visits and shelter support, with flexible day, evening, or weekend shifts.

Required Qualifications

  • Associate's, Bachelor's or Master's degree in Nursing from an accredited college or university
  • Current Registered Nurse licensure in the State of Colorado
  • Basic Life Support certification
  • Ability to work day, evening or weekend clinic as scheduled or appropriate
  • Ability to perform quick and accurate triage assessments with appropriate recommendations of care as outlined in SSHC nursing protocols via telephone or in-person
  • Ability to work 50% clinic based and 50% field based
  • Opportunity for either five 8 hour shifts or four 10 hour shifts

Desired Qualifications

  • Preferred experience in ambulatory, public, community, mental health, or emergency nursing
  • Familiarity with diverse or marginalized communities is highly desirable
  • Bilingual preferred
