Registered Nurse - Transfer Center Coordinator

Part-time Registered Nurse to serve as Transfer Center Coordinator at Trinity Health Grand Rapids. Coordinate EMS interfacility transfers for inpatient and emergency spaces, triage and prioritize transfers with Trinity Health EMS dispatch, and assist in prioritization of discharge transports to skilled nursing facilities, assisted living, and inpatient psychiatric care. Collaborate with Clinical Resource Coordinators on bed management and patient flow, collect and communicate critical clinical and logistical information, support ED-to-ED transfers, and participate in quality/process improvement initiatives (PDCA, LEAN).
