Job Summary
Conduct nursing oversight and case management for individuals in the Texas Home and Community-based Services (HCS) Program. Perform initial and ongoing nursing assessments, develop care plans, and manage RN delegation per Texas Board of Nursing rules. Monitor client health and safety, review physician orders, coordinate medical appointments and discharges, and provide training to Direct Support Professionals. Communicate with families, guardians, and interdisciplinary teams while maintaining accurate medical records. Participate in state surveys, audits, and on-call rotations as needed.
Required Qualifications
- Current Texas Registered Nurse (RN) license in good standing
- Valid Texas driver's license and reliable transportation
- Current CPR certification
- Excellent communication and organizational skills
- Strong computer skills including Microsoft Office
- Ability to work independently while collaborating with a multidisciplinary team
- Active Registered Nurse (RM) License in Texas, in good standing
- Minimum of 1-2 years of nursing experience
- Valid driver's license and reliable transportation
- Ability to travel within assigned caseload area
Desired Qualifications
- Experience working with individuals with Intellectual and Developmental Disabilities (IDD)
- Experience in the HCS or related waiver programs
- Knowledge of Texas HHS and HHSC regulations
- Experience with RN Delegation and BON requirements
- Case management experience
- IDD, long term care, or community health preferred
