Job Summary
Develop a patient plan of care that identifies interventions to achieve expected outcomes and evaluates progress toward attainment. Provide patient assessment and collection of health data involving the patient, family, and other health care providers. Communicate the plan of care to the patient and family regularly, respecting patient wishes through mutual goal setting. Communicate patient data to the physician and interdisciplinary team as indicated to achieve patient care needs. Document patient health data, interventions, and communications. Individualize patient and family teaching based on a thorough assessment to meet specific learning needs, including disease management strategies and drug information. Nurture a compassionate environment by providing psychological support and comply with HIPAA regulations.
Required Qualifications
- Associate's in Nursing
- Current State of Illinois Registered Nurse licensure
- Current CPR Certification issued by American Heart Association
