Job Summary
Registered Nurse Supervisor / Territory Growth Lead in New Haven County, CT. Initially functions as a working field case manager while building census and establishing the Nurses At Home presence; as the territory grows, transition to supervision, case management oversight, and clinical leadership. Responsibilities include providing skilled nursing care and case management for home health patients, wound care, COPD and cardiovascular management, diabetes education, medication management, admissions and recertifications, coordination of care, physician communication, patient education, and interdisciplinary collaboration. Lead territory growth efforts, build referral-partner relationships, onboard and supervise additional field case managers, participate in patient reviews and clinical oversight, and promote high-quality, patient-centered care. Requires active Connecticut RN license, minimum 2 years in home health or hospice case management, strong med-surg skills, ability to work independently in the field, excellent documentation/communication/organizational skills, leadership potential, and travel within New Haven County with valid driver’s license.
Required Qualifications
- Active Connecticut Registered Nurse license
- Minimum of 2 years of home health or hospice case management experience
- Strong medical-surgical clinical skills
