Registered Nurse - Suffolk

Conduct in-home assessments and develop personalized care plans for clients transitioning to AmeriCare Plus In-Home Personal Care Agency. Coordinate admissions paperwork and ensure compliance with agency policies while communicating with families, caregivers, and physicians to maintain continuity of care. Support clients with compassion and clinical expertise during the transition to in-home care throughout the Orange area of Virginia. This role offers part-time or full-time flexibility with a supportive team environment and meaningful work that changes lives.
