Job Summary
Registered Nurse (RN) providing home care services in the Stockton/Roseville, CA area. Responsibilities include completing initial patient and family assessments, regularly re-evaluating nursing needs, initiating and revising care plans, administering medications as prescribed, counseling patients and families, delivering health instructions, coordinating discharge planning, and acting as case manager when assigned. Additional duties include preparing clinical notes, communicating with physicians and community health partners, participating in on-call duties, and supervising home health aides. Requires NLN-accredited nursing education, active nursing license in California, and CPR certification. Must be a licensed driver with reliable vehicle.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state.
- Graduate of National League for Nursing accredited school of nursing.
- Maintain a current CPR certification.
