Job Summary
Assess, plan, implement, coordinate, delegate, and monitor tasks for assigned patients in the Transitional Care Unit, delivering age-appropriate care for a wide variety of medical conditions. Perform specialized nursing tasks including biophysical and psychosocial assessments, pain management, and abuse/neglect screening to develop and modify patient care plans. Collaborate with physicians and the care team while developing comprehensive family educational plans and ensuring timely documentation of physician orders. Maintain constant alertness and apply evidence-based practice to meet clinical needs, with a focus on safe patient handling and data entry. Requires a licensed new graduate nurse from an accredited program actively participating in the SSM Health New Graduate Nurse Residency Program.
Required Qualifications
- Licensed new graduate nurse from an accredited nursing program
- Active participation in SSM Health New Graduate Nurse Residency Program
- Graduate of accredited school of nursing or education equivalency for licensing
