Job Summary
Deliver exceptional nursing care using the nursing process, administer medications per physician orders, and interpret diagnostic tests to adjust care plans. Provide pain assessment and management while educating patients on treatment options. Coordinate care for chronic conditions by stratifying patient populations by risk and implementing care plans to reduce avoidable acute illness. Facilitate Annual Wellness Visits, Chronic Care Management encounters, and care transitions through daily huddles with providers. Conduct clinical health coaching to support self-management and mobilize community resources for obesity, tobacco cessation, and cardiovascular health. Maintain accurate documentation for compliance and billing while supporting a culture of safety and quality. This unique role combines direct patient care with care coordination responsibilities in Bridger Valley.
Required Qualifications
- Graduate of an accredited nursing program
- Current Wyoming RN license
- AHA Basic Life Support (BLS)
- PALS and NRP (or ability to obtain within 6 months)
Desired Qualifications
- Previous experience in chronic disease management and direct patient care
- Background in Care Coordination, Case Management, or Behavioral Health
- Familiarity with health IT systems and data reporting
