Job Summary
Implement the nursing process to promote and restore patient health while supervising assigned team members and collaborating with physicians. Establish patient care goals, teach self-care skills, and resolve complaints using multidisciplinary strategies. Monitor tracking systems for patient results, ensure referrals are processed per policy, and report clinical issues to the Practice Manager or Nurse Manager. Facilitate clinical orientation for new hires and coordinate care with providers. Participate in evening and Saturday rotations in a full-time outpatient setting.
Required Qualifications
- Valid and current RN License to practice in the State of Connecticut
- CPR certification
- Associate's Degree from an accredited registered nursing program approved by the State of Connecticut Department of Public Health
- Bachelor's Degree
- Minimum of one to two years of experience as an RN or prior experience as an LPN in a community health setting
- Bi-lingual English / Spanish
- Proficient in using EMR
- Knowledge of Microsoft Word, Excel and PPT
- Critical thinking skills
- Ability to create positive motivation
- Strong verbal and written communication skills
- Ability to work independently
- Ability to organize and manage multiple priorities and work under time pressure deadline
- Ability to demonstrate consistently sound judgment

- Must be able to lift 50 lbs
Desired Qualifications
- Familiarity with tele-medicine
- Prior experience with tele-medicine
- Bi-lingual English / Spanish (helpful)
