Job Summary
Deliver patient care within a team-based primary care model by performing assessments, administering medications, and educating patients on health and wellness. Coordinate care, referrals, and follow-ups using digital tools to support patient engagement while ensuring compliance with clinical standards and MTF policies. Respond to urgent and emergent patient needs and supervise delegation tasks as appropriate. Work Monday through Friday from 7:00 AM to 4:30 PM with no nights or weekends. This role supports the 28th Medical Group's Patient-Centered Medical Home model, providing whole-person, preventive care to military members and their families.
Required Qualifications
- U.S. Citizenship
- Graduate of an accredited nursing program (ADN or higher)
- Current, active, and unrestricted Registered Nurse license (South Dakota)
- BLS certification
- Minimum 1 year of nursing experience
- Must successfully complete a background investigation, fingerprinting, and a rigorous credentialing process to work on a military installation
- Must meet all applicable federal contract and clinical competency requirements
Desired Qualifications
- At least 2 years of experience in outpatient, ambulatory, or primary care settings
- Strong clinical skills with the ability to think critically in a fast-paced environment
- Experience using electronic health records (EHR)
- Excellent communication skills and a team-oriented mindset
- Military healthcare experience
