Assess residents on admission and when conditions change, documenting care plans and medical records. Administer medications following the five rights while evaluating clinical changes related to medical diagnoses. Educate residents and families on pain management and PPS documentation requirements. Supervise CNAs and LPNs to ensure continuity of care, delegate tasks to the healthcare team, and participate in interdisciplinary care planning. Accompany physicians on rounds, prepare 30 and 60-day reviews, and notify medical staff of condition changes. Provide thorough shift reports via walking rounds, address resident complaints, and advocate for resident rights and dignity. Comply with all applicable regulations and facility policies.
