Job Summary
Deliver clinical assessments, problem-solving, and complex care training to direct care workers for clients across Warren Blackwood, Donnybrook, and Pemberton. Provide personal care, educate families, and manage documentation using computer literacy skills while working autonomously within a team focus. Coordinate service provision and compliance with contractual obligations in a community environment, ensuring safe, personal, and effective care standards. This part-time role (3 days per week, 8am–4pm) offers professional development, study leave, and a vehicle subsidy. Silverchain, an accredited not-for-profit home care provider, delivers hospital-standard care to 140,000 clients annually.
Required Qualifications
- Registered Nurse
- Demonstrated understanding of healthcare in a community environment, including service provision, coordination, allocation and compliance with contractual obligations
- Skills in the area of clinical assessment, problem solving, client and family education
- Ability to demonstrate personal care and provide competency based complex care training to direct care workers
- Well developed communication (written, verbal and listening) and interpersonal skills to develop effective relationships, promote services and resolve client/staff issues
- Demonstrated decision making and problem solving skills to innovatively manage the provision of quality care
- Demonstrated computer literacy in word processing, spreadsheets, and calendar management software to prepare correspondence and effective documentation
- The ability to work autonomously with a team focus to contribute to business unit and organisational objectives
- Part time (3 days per week)
- Working hours will be 8am - 4pm
- Days of work between Monday to Friday only
