Job Summary
Conduct health assessments, develop individualized care plans, and administer medications to adults and seniors in the Adult Day Program. Maintain nursing documentation, monitor participant health, and proactively address medical concerns while promoting health education and preventative care. Provide training to staff and collaborate with physicians, families, and interdisciplinary teams to ensure coordinated care outcomes. Participate in care planning meetings and help maintain a safe, person-centered environment for all participants.
Required Qualifications
- Graduate of an accredited Registered Nurse (RN) program
- Current Massachusetts RN license
- Current CPR and First Aid certification
Desired Qualifications
- One year of nursing experience, particularly with medically complex adults, seniors, or individuals with disabilities
- Strong communication, organizational, and critical-thinking skills
- Bilingual candidates, especially those fluent in Spanish
