Job Summary
Conduct medical case management and Critical Time Intervention support for clients transitioning from hospitals to the community. Perform medical assessments, coordinate medication delivery and administration, and execute physician-prescribed regimens while ensuring client compliance. Provide health education, counseling, and crisis intervention to individuals and groups, including urine screenings and referrals for required medical services. Maintain accurate documentation of all medical services rendered and communicate relevant resident information to supervisors. Complete timely assessments within mandated time frames and observe self-administration protocols for detoxification.
Required Qualifications
- NYS RN license
- Knowledge of chemical dependency as a primary diagnosis
- Excellent written and verbal communication
- Assessment and case management
- Knowledge of Microsoft Office tools
- Ability to provide crisis intervention utilizing good interpersonal skills with a high level of maturity and responsibility
- Monday - Friday 9 am - 5p
- In Person
Desired Qualifications
- Basic knowledge of human services
- Bilingual (Spanish/English) speaking preferred
