Job Summary
Provide hands-on clinical support for seniors across individual homes and various senior living communities, including care assessments, development and implementation of chronic condition care plans, coordination with primary care providers and families, and comprehensive documentation in EMR/EHR systems. Collaborate with on-site leadership and internal team members to ensure seamless resident care, offer after-hours triage support, and travel between assigned senior living settings and homes as part of a growing startup offering.
Required Qualifications
- Active Idaho RN license
- Valid driver's license
- Strong background in geriatric/senior care
- Experience in care planning for chronic conditions with interdisciplinary teams
- Excellent documentation and EMR/EHR skills
