Job Summary
Conduct head-to-toe assessments, triage urgent clinical situations, and manage acute and post-acute care for clients in senior living communities across York County. Perform comprehensive wound assessments, medication reconciliation, teaching, and safety monitoring while documenting care at the point of care. Work independently to make real-time clinical decisions in varied home environments, prioritizing urgent visits and adapting to changing patient needs. Requires a current Pennsylvania nursing license and at least one year of hands-on experience in adult/geriatric acute or home health settings.
Required Qualifications
- A current nursing license in good standing in the state of Pennsylvania
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings)
Desired Qualifications
- Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
- Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
- Triage and manage urgent clinical situations independently in the home setting
- Perform medication reconciliation, teaching, and safety monitoring
- Perform comprehensive wound assessment, care, and documentation
- Ability to work independently while knowing when to escalate concerns
- Comfortable making real-time clinical decisions without immediate on-site support
- Strong problem-solving skills in unpredictable home environments
- Ability to manage a dynamic visit schedule and prioritize urgent visits
- Efficient with point-of-care documentation and technology
