Job Summary
Conduct comprehensive client assessments of physical, mental, and psychosocial needs to develop individualized care plans. Collaborate with families and the care team to create and update personalized care plans, then monitor and coordinate services to ensure adherence while making adjustments for changing needs. Provide guidance and mentorship to caregivers regarding care plans and best practices, while maintaining accurate records of all assessments and condition changes. Ensure all care practices comply with federal, state, and local regulations alongside company policies. This role requires an active RN license in South Carolina with at least one year of experience in long-term or home health care.
Required Qualifications
- Active Registered Nurse (RN) license in the state of South Carolina
- Minimum of one year of experience in long-term care or home health care settings
- Strong assessment, critical thinking, and problem-solving skills
- Excellent communication and interpersonal abilities, with a compassionate approach to client care
- Ability to work independently and as part of a collaborative team
- Proficiency in electronic health records (EHR) and other relevant software
Desired Qualifications
- Bachelor of Science in Nursing (BSN) preferred
