Job Summary
Conduct nursing assessments and observations at client homes in Brooklyn Park, Minneapolis, and surrounding areas. Evaluate clients regularly to ensure care plans remain appropriate and develop, monitor, and update individualized care plans in collaboration with the team. Provide clinical guidance, education, and ongoing support to caregivers while addressing client concerns and coaching staff on safety procedures. Mentor caregivers to promote professionalism and excellence in caregiving standards. Maintain active RN licensure and BSN, with a valid driver's license required for commuting.
Required Qualifications
- Bachelor of Science in Nursing (BSN)
- Current and active RN license
- Maintain CEU requirements per Board of Nursing standards
- Valid driver's license, auto insurance, and a reliable vehicle to commute to work
Desired Qualifications
- Minimum of 1 year of home health experience preferred
- Comfortable using technology and software systems (WellSky, Microsoft Office 365 a plus)
- Experience supervising or mentoring caregivers preferred
