Job Summary
Full-time Registered Nurse Case Manager (RN) serving geriatric patients via individualized care plans with a hybrid schedule (Monday–Friday, 8:00am–4:00pm; 3 days in the community, 2 days at home; no nights or weekends). Responsibilities include helping patients set up services, seeing patients in their homes, and completing admin tasks from home; no direct bedside patient care. Requires MA RN license, 2+ years of nursing experience, CPR certification, and strong communication skills with an interdisciplinary team.
