Job Summary
Perform assessments of physical, emotional, psychosocial, and environmental needs of patients and report changes to the primary Community Health Nurse and physician. Assist in establishing and evaluating the comprehensive Plan of Care with patient and caregiver participation, while providing education to families regarding condition knowledge deficits. Maintain accurate clinical records per policy and supervise ongoing education for Home Health Assistants and Personal Care Assistants. Receive assignments from a supervisor to complete all visits, participate in case conferences, and perform related duties as required. Requires a current New York State RN license, one year of clinical assessment experience, and on-call duty with hours determined by patient need.
Required Qualifications
- Registered Professional Nurse (RN)
- current New York State license and registration
- Graduate from an accredited school, university or college of Registered Professional Nursing
- A Baccalaureate Degree in Nursing from an approved program
- A diploma from a New York State accredited RN Hospital Diploma Program
- An Associate's degree in nursing from an accredited program
- At least one year previous medical/surgical or clinical assessment experience
Desired Qualifications
- Experience as a Home Care Nurse (CHHA, LTHHCP, LHCSA or Hospice)
