Job Summary
Assess health status of clients assigned to a daily work schedule and develop a plan of care. Perform and document nursing intake and medical history, triage sick calls, and make referrals to appropriate medical practitioners. Complete follow-up assessments for chronic and acute illnesses, screen clients for TB via PPD administration, and monitor lab values to report abnormalities. Execute prescribed treatments and diagnostic testing, draw blood specimens, and review medical records to ensure continuity and identify transfer needs. Requires NYS RN licensure, BLS certification, and an Associate's Degree in Nursing with 1–3 years of experience. Flexible schedule availability is required.
Required Qualifications
- NYS RN licensure
- BLS certification
- Associate's Degree in Nursing
- Minimum of One-Three (1-3) year demonstrated experience as an RN preferably working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI)
- Available to work a flexible schedule as needed in response to participant and staff needs
- Computer literacy including proficiency in Microsoft Office Suite and EMR
- Ability to work under pressure
- Ability to put patients at ease and explain physician's instructions
- Ability to multi-task with excellent organization skills
- Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds
- Knowledge of medical terminology
- Team player and ability to work independently
