Job Summary
Assess health status of assigned clients and develop care plans. Perform and document nursing intake and medical histories. Triage sick calls and refer clients to appropriate medical practitioners. Complete follow-up assessments for chronic and acute illnesses to monitor treatment effectiveness. Screen clients for tuberculosis by administering PPD and assessing reactions. Monitor lab values, identify abnormalities, and report to physicians. Review medical records to ensure continuity of care and identify transfer needs. This role requires NYS RN licensure, an associate's degree, and one to three years of experience, preferably with clients facing substance use or serious mental illness. Flexible scheduling is available based on participant and staff needs.
Required Qualifications
- NYS RN licensure
- BLS certification
- Associate's Degree in Nursing
- Minimum of One-Three (1-3) year demonstrated experience as an RN preferably working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI)
- Available to work a flexible schedule as needed in response to participant and staff needs
- Computer literacy including proficiency in Microsoft Office Suite and EMR
- Ability to work under pressure
- Ability to put patients at ease and explain physician's instructions
- Ability to multi-task with excellent organization skills
- Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds
- Knowledge of medical terminology
- Team player and ability to work independently
