Job Summary
Provide in-home nursing care for clients, including one-on-one home visits, performing initial and ongoing assessments of physical, psychosocial, and environmental needs; monitor and document patient conditions (vital signs, medication review); administer prescribed medications; educate patients and families on home health care strategies and devices to improve quality of life; communicate effectively with patients, families, and healthcare team; maintain flexible scheduling and travel within the local area to visit patients.
Required Qualifications
- Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing
- Current state license as an RN
- Proof of current CPR certification
- Current driver’s license and reliable transportation
- Ability to work a flexible schedule and travel locally
