Job Summary

Conduct comprehensive patient assessments, develop individualized care plans, and deliver specialized wound care treatment for long-term care residents. Perform hourly rounding, coordinate discharge planning, and delegate nursing tasks while collaborating with physicians and therapists. Participate in quality improvement initiatives, peer reviews, and Nurse Sensitive Indicators activities to support clinical excellence. Maintain current evidence-based practices and adhere to LifeBridge Health policies. Requires current Maryland RN license, BLS certification, and LTC wound care experience. Part-time day shift rotation with weekend requirements.

Required Qualifications

  • Current Maryland RN License or eligibility to obtain Maryland licensure
  • Basic Life Support (BLS) Certification
  • Associate Degree in Nursing (ADN)
  • Long-Term Care (LTC) Wound Care experience
  • Strong assessment, clinical judgment, and communication skills
  • Weekend Requirement: Every 3rd weekend rotation

Desired Qualifications

  • Bachelor of Science in Nursing (BSN)
  • Previous experience in skilled nursing, rehabilitation, or post-acute care settings
  • Wound Care Certification (WCC, WOCN, or similar)
