Job Summary
Facilitate safe and timely transfers of patients with emergent, urgent, or medically complex conditions by performing triage assessments and collaborating with physicians and transport staff. Act as a liaison between referring and receiving facilities using closed-loop communication to coordinate transfer requests and document all activities in the electronic medical record. Prepare monthly reports on transfer center trends and participate in quality improvement activities to enhance patient outcomes. Requires an ADN and three years of nursing experience, with 24 hours per week split into two 12-hour weekend shifts from 7a-7p. Pay ranges from $38.20 to $57.30.
Required Qualifications
- ADN (associate's degree in nursing)
- Licensed Registered Nurse (RN) in relevant state
- Minimum 3 years of nursing experience
- Must be able and available to work on-site
- Strong verbal and written communication skills
- Proficiency in using electronic medical records and telehealth technology
- Excellent critical thinking and problem-solving skills
- Ability to work efficiently under pressure and manage multiple tasks simultaneously
- Strong interpersonal skills to effectively collaborate with healthcare team
- Must be available for 2, 12 hour weekend shifts per week
- 7a-7p schedule availability
Desired Qualifications

- Certified Acute Care Virtual Registered Nurse (CAVRN)
