Job Summary
Conduct comprehensive assessments of patients and families to determine hospice needs, utilizing the nursing process to write and revise care plans. Administer medications and treatments as prescribed, counsel patients on self-care techniques, and assist with activities of daily living. Coordinate patient care as a case manager by communicating with physicians, community health persons, and the interdisciplinary group to ensure continuity of emotional and physical support. Maintain accurate clinical notes documenting pain management and condition changes while participating in quality assessment and performance improvement teams. Requires 1–2 years of acute and home care experience, current state licensure, and a Bachelor's degree preferred. Full-time role with weekend on-call duties, $75k–$82k annual pay, and benefits including 401k with 100% match and PTO accrual starting immediately.
Required Qualifications
- Graduate of an accredited school of nursing
- One (1) to two (2) years of recent acute care experience in an institutional setting
- One (1) to two (2) years of recent experience in home care
- Current licensure in State
- Licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Bachelor's degree
- One (1) year of hospice care experience preferred
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Prolonged or considerable walking or standing
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping bending, kneeling and/or crouching
- Visual acuity and hearing to perform required nursing skills
Desired Qualifications
- Bachelor's degree, with one (1) year of hospice care experience preferred
