Job Summary
Perform weekend home health and hospice visits, acting as the primary point of contact for all disciplines involved in patient care. Coordinate client care episodes, consult with physicians and the office regarding patient details, and ensure accurate OASIS collection, up-to-date medication profiles, and timely lab value reporting. Attend weekly case conferences and monthly case manager meetings while maintaining current medication and lab profiles. This hybrid role requires Saturday and Sunday availability and full-time status, offering broader clinical experience across both care settings for licensed nurses with at least one year of clinical experience.
Required Qualifications
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- Possess a valid state driver's license and automobile liability insurance
- Saturday and Sunday are required
- Full-time weekend RN position
Desired Qualifications
- A minimum of one year of clinical experience is preferred
