Job Summary
Conduct comprehensive physical, psychosocial, emotional, and spiritual assessments to function as interventionalists in the home. Develop patient care plans with physicians and interdisciplinary teams for Start of Care visits, post-hospital evaluations, re-certifications, discharges, and supervisory visits. Implement clinical protocols during admission visits and make resource utilization recommendations to achieve positive outcomes. Initiate and respond to requests for care plan changes, determining if Significant Change in Condition triggers an OASIS requirement. Provide thorough disease management education and instruct patients, families, and caregivers on self-care and community resources. Requires an active Registered Nurse license and 1-3 years of home health or hospice experience.
Required Qualifications
- Registered Nurse (RN) - Board of Nursing
- An active license in the state of practice Upon Hire
- The ability to work the hours specified
- Visual acuity adequate to perform position duties
- The ability to communicate effectively with others
- The ability to hear, understand and distinguish speech and other sounds
