Job Summary
Conduct comprehensive physical, psychosocial, emotional, spiritual, and bereavement patient assessments to function as an interventionalist in the home. Implement the nursing process for Start of Care visits, post-hospital evaluations, re-certifications, discharges, and supervisory visits while collaborating with physicians to develop patient-centered care plans. Initiate and respond to requests for changes in care plans, determining if Significant Changes in Condition require an OASIS. Provide thorough instruction and disease management education to patients and caregivers regarding the disease process and community resources. Partner with the interdisciplinary team to optimize resource utilization and ensure efficient care delivery.
Required Qualifications
- Registered Nurse (RN)
- Active license in the state of practice upon hire
- Ability to work the hours specified
- Visual acuity adequate to perform position duties
- Ability to communicate effectively with others
- Ability to hear, understand and distinguish speech and other sounds
Desired Qualifications
- 1-3 years in home health or hospice
