Job Summary
Provide skilled home health nursing including wound care, medication administration, and chronic disease management in the patient’s home. Deliver patient-centered care, educate patients and caregivers on disease processes and regimens, and ensure accurate, compliant documentation in the EHR. Coordinate care with physicians, case managers, and interdisciplinary teams; participate in Medicaid audits and quality assurance activities; advocate for patient needs and assist with accessing community resources. Frequent travel to patient homes across assigned service areas; flexible schedule including evenings or weekends based on patient needs.
Required Qualifications
- Current and unrestricted Registered Nurse (RN) license in the state of Indiana
- Minimum of 1 year of nursing experience; home health and Medicaid experience strongly preferred
- Knowledge of Indiana Medicaid policies, documentation standards, and prior authorization processes
- Current CPR certification
- Valid driver's license and reliable transportation
- Strong clinical judgment, communication, and organizational skills
- Experience with electronic documentation systems preferred
