Job Summary
Registered Nurse (RN) for Home Health in Walkerton, IN. Deliver skilled nursing services including wound care, medication administration, chronic disease management, and health education in patients' homes. Ensure services meet Medicaid reimbursement standards, document in the EHR, coordinate care with physicians and care teams, educate patients/caregivers, participate in Medicaid audits and quality assurance, advocate for patient needs, and assist with accessing community resources. Flexible schedule with frequent travel to patient homes across assigned service areas.
Required Qualifications
- Current and unrestricted RN license in Indiana
- Minimum of 1 year nursing experience; home health and Medicaid experience strongly preferred
- Knowledge of Indiana Medicaid policies, documentation standards, and prior authorization processes
- Current CPR certification
- Valid driver’s license and reliable transportation
