Job Summary
Registered Nurse (RN) needed for Home Health in Winamac, IN. Responsibilities include delivering skilled nursing services (wound care, medication administration, chronic disease management, health education), ensuring services meet Medicaid reimbursement standards, coordinating care with physicians and care teams, educating patients and caregivers, maintaining accurate EHR documentation, participating in Medicaid audits and quality assurance, and advocating for patient needs and community resources. Requires Indiana RN license, 1 year nursing experience (home health/Medicaid preferred), CPR certification, valid driver’s license, reliable transportation, and strong clinical judgment and communication. Travel to patient homes across the service area with flexible scheduling.
Required Qualifications
- Current and unrestricted Registered Nurse (RN) license in the state of Indiana
- Minimum of 1 year of nursing experience; home health and Medicaid experience strongly preferred
- Knowledge of Indiana Medicaid policies, documentation standards, and prior authorization processes
- Current CPR certification
- Valid driver’s license and reliable transportation
- Strong clinical judgment, communication, and organizational skills
- Experience with electronic documentation systems preferred
