Job Summary
Provide nursing care to home hospice patients following the ANA Standards of Clinical Nursing Practice. Utilize the nursing process to collect health data, assess, diagnose, plan, implement, and evaluate patient outcomes. Develop and implement individualized care plans, collaborate with patients, families, and healthcare team, and ensure safety and confidentiality in home or facility settings. Requires on-call approximately 2 times per month; serves a 40-mile radius of Sioux Falls and 30-mile radius of Madison and Flandreau. Minimum two years related experience preferred.
