Registered Nurse (RN) Visiting Home Health

Conduct home health visits to administer services, perform comprehensive assessments, and update patient treatment plans. Administer IVs, change dressings, clean wounds, and coordinate care with the Clinical Team Manager, physician, and other team members. Document all findings and upload data to the Synergy Software portal while obtaining and revising physician orders. Supervise Home Health Aides, manage patient transfers and discharges, and assess needs for physical, occupational, and speech therapy services. Maintain knowledge of regulatory requirements and demonstrate strong communication skills throughout the patient's journey.
