Job Summary
Administer at-home IVs, change dressings, clean wounds, and perform comprehensive patient assessments during home health visits. Document all findings and upload data to the Synergy Software portal while obtaining and verifying physician orders. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise patient care plans, coordinating significant changes with physicians and other team members. Execute re-certifications, transfers, and discharges as directed, and conduct Home Health Aide supervisory visits per regulatory requirements. Maintain knowledge of state regulations and communicate patient status updates to the agency. Requires current CA RN license, BLS certification, and IV/Phlebotomy credentials.

- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Required)
- able to work at least two-three days each week (or ten productivity units)
- one weekend a month
- two holidays per year (one winter and one summer)
