Job Summary
Conduct home health visits to administer services, perform comprehensive assessments, and update physician orders. Manage patient treatment plans, coordinate care with the Clinical Team Manager and other specialists, and handle re-certifications, transfers, and discharges. Supervise Home Health Aides and document all activities via Synergy Software, ensuring compliance with regulatory requirements. Maintain IV and wound care skills while communicating significant patient changes to the team. Requires at least one year of RN experience, a current CA license, and BLS certification. Flexible scheduling includes weekend and holiday coverage.

- Ability to work one weekend a month
- Ability to work two holidays per year
- Visiting RN for Home Health experience for 1 year
- 1 year of Nursing experience
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
