Registered Nurse (RN) Visiting Home Health

Conduct home health visits to administer services, perform comprehensive assessments, and update physician orders. Change dressings, clean wounds, and oversee treatment plans while documenting findings and uploading data to the Synergy Software portal. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise patient care plans, coordinating significant changes with physicians and other team members. Manage re-certifications, transfers, and discharges, and supervise Home Health Aides per regulatory requirements. Maintain current CA RN licensure, BLS certification, and IV/Phlebotomy credentials. Work flexible schedules including weekends and holidays to support 7-day patient care needs across Bakersfield, Lancaster, and So-Cal.
