Registered Nurse (RN) Visiting Home Health

Conduct home health visits to administer IVs, change dressings, clean wounds, and perform comprehensive patient assessments. Document all findings, upload records to the Synergy Software portal, and obtain physician orders. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise patient care plans, while coordinating significant condition changes with physicians and other team members. Execute re-certifications, resumption of care, transfers, and discharges as directed, and supervise Home Health Aides per regulatory requirements. Maintain current CA RN license, BLS certification, and IV/Phlebotomy credentials. Work flexible schedules including weekends and holidays, earning $50–$140 per visit.
