Job Summary
Conduct home health visits to administer services, perform comprehensive assessments, and update patient treatment plans. Administer at-home IVs, change dressings, clean wounds, and document all findings in Synergy Software. Obtain physician orders, coordinate care with the Clinical Team Manager, and initiate or revise plans of care. Supervise Home Health Aides, assess needs for additional team members, and manage re-certifications, transfers, and discharges. Maintain regulatory compliance and communicate significant patient changes to physicians and the agency. Requires current CA RN license, BLS certification, and IV/Phlebotomy skills. Flexible scheduling available for two to three days weekly with weekend and holiday rotations.
Required Qualifications
- 1 year RN experience
- 1 year nursing experience
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
- Ability to work at least two-three days each week
- Ability to work one weekend a month
- Ability to work two holidays per year (one winter and one summer)
- Nursing documentation/charting skills
- Fast learner
- Great communication skills
- Highly motivated
- Positive-minded
- Knowledge of regulatory requirements
